​ 34 yr male ,farmer by occupation came to casualty with


C/o - patient complains about burning micturition -7days

No urine output- 3 days

SOB -2 days


HOPI-

Patient was apparently asymptomatic 7days back then he developed burning micturition and no urine output since 3 days.

Pt lives in well lit house in vellanki

He wakes up at 4 and off to work at 5

Patient takes rice three times a day.


PAST HISTORY:-

No similar complaints in the past

No hypertension 

No diabetes 


PERSONAL HISTORY:-

No loss of appetite

Diet- mixed

Bowel and bladder movements 

Sleep: adequate 

Addictions: patient drinks alcohol on occasions

No smoking habits


SOCIOECONOMIC STATUS-

Patient lives in vellanki,yadadri in ventilated house


FAMILY HISTORY :-

No member of family has similar disease


DRUG HISTORY:-

No H/o allergic known drugs


GENERAL EXAMINATION:-

Patient is moderately fit and nourished 

Cooperative 

Conscious 


No c/o of giddiness

No c/o of loose stools 


No pallor 

No icterus

No cyanosis

No clubbing

No generalised lymphadenopathy 


VITALS:-

Temp:-Afebrile(98.4F)

RR :  16cycles/min

PR:- 120 beats/min

BP:-150/70mmhg


SYSTEMIC EXAMINATION:-

RS:-BAE  

position of trachea - central

No drooping of shoulders

Chest- bilaterally symmetrical

CVS:-S1 S2 no murmurs heard

ABDOMEN:- distended 

CNS:-NAD

Patient is conscious .His speech is normal
